By participating in any event hosted by Life's A Beach Volleyball Club, you agree to the following terms listed below:

Waiver and Release of Liability

​

By submitting the online RSVP form, the participant gives consent to participate in this Life's A Beach-sponsored program. I, the participant, hereby release, discharge, and otherwise indemnify Life's A Beach Volleyball Club, Dan Palenchar, Joshua Daguman, Michael Walsh, City of San Diego, San Diego County California, the State of California, directors, managers, employees, contractors, agents, volunteers and officials, against any claim by or on behalf of the participant.  I, the participant, take full responsibility for any injuries incurred or resulting in death. In the event of an emergency, call 911. At the discretion of Life's A Beach Volleyball Club, care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of the participant. I, the participant, understand that insurance will not be furnished by Life's A Beach Volleyball Club, City of San Diego, San Diego County California, or the State of California.

By submitting the RSVP form(s), the participant acknowledges that volleyball or any sporting event is an extreme test of a person’s physical and mental limits and that participation in a volleyball event can cause death, serious injury, or property damage.  With a full understanding of the potential risks, I, the participant, hereby understand the risks of participating in volleyball or any sporting event. I, the participant, acknowledge that additional information may be received regarding additional Life's A Beach Volleyball Club-sponsored programs.

Covid-19 Release of Liability

​

The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Life's A Beach Volleyball Club has put in place preventative measures to reduce the spread of COVID-19; however, cannot guarantee that the player, their family and/or guests will not become infected with COVID-19. Further, attending any Life's A Beach Volleyball Club program or activity could increase the risk of contracting COVID-19.

By submitting the registration form(s), the participant acknowledges the contagious nature of COVID-19 and voluntarily assumes the risk that the participant and I may be exposed to or infected by COVID-19 while attending and/or participating in the Life's A Beach Volleyball Club or activity and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I, the participant, understand that the risk of becoming exposed to or infected by COVID-19 at the Life's A Beach Volleyball Club program or activity may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Life's A Beach Volleyball Club employees, volunteers, and program participants and their families.

​I, the participant, voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, permanent disability or death), illness, damage, loss, claim, liability, or expense, of any kind, that I, the participant, may experience or incur in connection with my attendance at the Life's A Beach Volleyball Club program or activity.

By submitting the RSVP form(s), the participant hereby releases, covenant not to sue, discharges, and holds harmless Life's A Beach Volleyball Club, City of San Diego, San Diego County, and the State of California, its volunteers, employees, agents, and representatives, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I, the participant, understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Life's A Beach Volleyball Association, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after attending any Life's A Beach Volleyball Club program or activity.

Pictures and Videos Release Acknowledgement

I, the participant, by submitting the online RSVP form(s) give consent to Life's A Beach Volleyball Club to use pictures and/or videos taken of the participant for use in educational review, social media and/or promotional purposes.
